Services we provide
Our services are available days, evenings, and nights every day throughout the
week. We are able to provide care for any duration of time.
Licensed Residential Supports
• Provided in single-family homes that are licensed by the state
and owned or leased by Open Arms Health Care, LLC
• Supervision available 24 hours a day, 7 days a week, 365 days
a year
• Includes medication administration, meal planning,
housekeeping, activity planning, and transportation
• OAHC will transport service recipients to all medical and
therapeutic appointments
• Ideal for individuals with Consolidated Waiver through the
Office of Developmental Programs

In-Home & Community Supports
• Provided in a service recipient’s personal home on a scheduled
basis according to the individual’s needs
• Staff members teach the individual about meal planning, home
maintenance, personal care, and other activities of daily living
• A client’s family members can be hired to provide this service
• This service is goal oriented with an emphasis on fostering
independence
• Ideal for PFDS/Community Living/Consolidated Waiver
individuals who live on their own or with their families and
need assistance with developing their skillset

Behavior Supports
• Provided by highly trained or licensed behavior specialists who
work in an individual’s own home or one of OAHC’s licensed
residential sites
• Each service recipient receives a Functional Behavior
Assessment and a Behavior Support Plan
• Includes ongoing support services that are designed to help
manage behaviors
• Can be used to train caregivers in best practices to manage an
individual’s needs
• Ideal for individuals who have behavioral challenges or are
looking to improve their coping skills
Companion Service
• Provided in an individual’s private home on a scheduled basis
according to need
• Services are focused on health and safety and ensuring that the
individual is supervised
• Staff members supervise the individual during personal care
and community activities
• A client’s family members can be hired to provide this service
• Ideal for PFDS/Community Living/Consolidated Waiver
individuals who live on their own or with their families and
need close supervision as they go about their daily lives

Respite Service
• Provided to individuals who receive at least one other service
through Open Arms Health Care, LLC
• Respite provides temporary relief for family members who
supervise their loved ones. The client can come to an OAHC
site or OAHC can provide services in an individual’s own home
while the primary caregiver is away
• Ideal for PFDS/Community Living/Consolidated Waiver
individuals who live with their families

Chore Service
• Provided in an individual’s private home on a scheduled basis

• Services are focused on maintaining the individual’s
environment so it is clean and safe
